چکیده

Smoking induced changes in the secretory cells of bronchiolar epithelium by facilitating secretion of cross linked glycoprotein mucus may influence the efficiency of mucus-cilia coupling. The functional impact on mucociliary transport in small (peripheral) airways has been studied by comparing data on aerosol deposition and clearance from symptomless cigarette smokers (30 tests, 18 subjects) with data from age matched non-smokers (30 tests, 19 subjects). Gamma camera images, assessed in terms of a penetration index comparing peripheral with inner zone deposition, indicated closely similar initial deposition in the two groups. Alveolar deposition, however, assessed in terms of particle retention at 24 hours, was significantly (p less than 0.01) less in the smokers. Given the similarity of initial deposition, this implies that an increased proportion of small conducting airways are protected by mucociliary defence in the smokers' lungs. Clearance from conducting airways of the peripheral zone in tests with relatively high peripheral deposition (14 tests on smokers, and 12 on non-smokers) nevertheless proceeded at the same rate in smokers as in non-smokers.

Response and acclimatisation of symptomless smokers on changing to a low tar, low nicotine cigarette.

Ten symptomless smokers were switched from their usual cigarette to a low tar, low nicotine test cigarette for two weeks to investigate their immediate response and subsequent acclimatisation to the test cigarette. The tar (T) and nicotine (N) yields of the test cigarettes were T = 3.8 mg, N = 0.6 mg; the median yields of the usual cigarettes were T = 16.4 mg, N = 1.4 mg. The subjects were moni...

Evaluation of the risk of lung cancer associated with NAD(P)H: quinone oxidoreductase 1 (NQO1) C609T polymorphism in male current cigarette smokers from the Eastern India

NAD(P)H: quinone oxidoreductase 1 (NQO1) is an endogenous cellular defence mechanism against several carcinogenic quinones derived from cigarette smoke. NQO1 C609T polymorphism is a strong determinant of NQO1 structure and function. The people with mutant allele for this polymorphism has significantly reduced NQO1 activity. In this study, we tried to evaluate the risk of lung cancer as...

Bronchiolar aerosol deposition and clearance.

We have often, and over a long time, thought of the "small airways" as a mystery zone or a quiet zone [1]. Relative to central bronchial generations, the total crosssectional area of each bronchiolar generation is large. Airflow resistance should in consequence be low. Considerable changes due to disease could, thus, occur with little chance of being detected by conventional lung function testi...

Beta Defensin-2 Is Reduced in Central but Not in Distal Airways of Smoker COPD Patients

BACKGROUND Altered pulmonary defenses in chronic obstructive pulmonary disease (COPD) may promote distal airways bacterial colonization. The expression/activation of Toll Like receptors (TLR) and beta 2 defensin (HBD2) release by epithelial cells crucially affect pulmonary defence mechanisms. METHODS The epithelial expression of TLR4 and of HBD2 was assessed in surgical specimens from current...

Disease of the airways in chronic obstructive pulmonary disease.

The pathological hallmarks of chronic obstructive pulmonary disease (COPD) are inflammation of the small airways (bronchiolitis) and destruction of lung parenchyma (emphysema). The functional consequence of these abnormalities is airflow limitation. Airway abnormalities and emphysema interact in a complex fashion in the development of airflow limitation in COPD. In an attempt to improve underst...
